looks like a cluster of whiteheads

I have whiteheads, or so it looks like whiteheads but large. It gets inflamed and is hard. But it never will extract. Now I am noticing that these so called whiteheads are is clustering together as if to form a layer of white bumps under the skin. I have had them for a long time. There are some small since ones that looks like it can be extracted but won't budge. Whatever it is, when poked with a needle right on the white part, it does not hurt.
